Terms of Agreement

Operation Fit 4 Life (OFFL) is a program designed to help reduce stress, increase fitness and improve the overall health of its participants. The overall program will last for twelve months and is divided into four separate missions: Memphis, Gatlinburg, Branson and Asheville. Mission: Gatlinburg runs March 30, 2008 through June 21, 2008.

1. In order to complete the Mission, each participant must log 48 hours of fitness with a minimum of 30 hours at participating MWR facilities during the above mentioned time frame. Participating MWR facilities for OFFL include: Clarksville Base Physical Fitness Center, Fratellenico Physical Fitness Center, Gerstch Physical Fitness Center, Lozada Physical Fitness Center, Olive Physical Fitness Center, Freedom Fighters Physical Fitness Center, Estep Wellness Center, Gardner Indoor Pool, Outdoor Recreation, and Cole Park Golf Club.

2. Participants are required to log hours of all fitness activity completed.  Log forms can be downloaded from www.fortcampbellmwr.com/fit4life.  Forms must be submitted to the Sports Office (via email or hard copy) on the following dates for verification purposes:
            a.  May 5, 2008 – for all hours logged in April.
            b.  June 5, 2008 – for all hours logged inMay.
            c.  July 5, 2008 – for all hours logged through June 21. 

3. Failure to submit log forms could result in the participant being eliminated from the competition.

4. Those participants who meet all requirements will be entered in a random drawing.  The winning participant will receive a two night hotel stay, four tickets toDollywood, and $250 spending cash courtesy of Saturn of Clarksville. 

5. All individuals are eligible to participate.  Only Active Duty ID cardholders and their immediate dependents ages 18 and over are eligible for the prizes.  The winning participant is subject to verification.
